Background: Clinical genomic implementation studies pose challenges for informed consent. Consent forms often include complex language and concepts, which can be a barrier to diverse enrollment, and these studies often blur traditional research-clinical boundaries. There is a move toward self-directed, web-based research enrollment, but more evidence is needed about how these enrollment approaches work in practice. In this study, we developed and evaluated a literacy-focused, web-based consent approach to support enrollment of diverse participants in an ongoing clinical genomic implementation study. (...) Methods: As part of the Cancer Health Assessments Reaching Many (CHARM) study, we developed a web-based consent approach that featured plain language, multimedia, and separate descriptions of clinical care and research activities. CHARM offered clinical exome sequencing to individuals at high risk of hereditary cancer. We interviewed CHARM participants about their reactions to the consent approach. We audio recorded, transcribed, and coded interviews using a deductively and inductively derived codebook. We reviewed coded excerpts as a team to identify overarching themes. Results: We conducted 32 interviews, including 12 (38%) in Spanish. Most (69%) enrolled without assistance from study staff, usually on a mobile phone. Those who completed enrollment in one day spent an average of 12 minutes on the consent portion. Interviewees found the information simple to read but comprehensive, were neutral to positive about the multimedia support, and identified increased access to testing in the study as the key difference from clinical care. Conclusions: This study showed that interviewees found our literacy-focused, web-based consent approach acceptable; did not distinguish the consent materials from other online study processes; and valued getting access to testing in the study. Overall, conducting empirical bioethics research in an ongoing clinical trial was useful to demonstrate the acceptability of our novel consent approach but posed practical challenges. (shrink)

Most relational theories assert both that spatial discourse is reducible to talk about physical objects and their spatial relations, and that the relation of congruence derives from a non-metrical relation which intervals bear or possibly bear to measuring instruments. We have shown that there are serious logical difficulties involved in maintaining both these positions and the thesis of the continuity of space. We have also shown that Grünbaum's motivating argument for the reduction of congruence is unsound, and, moreover, that the (...) prospects of formulating the kind of definition which this reduction requires seem to rest upon a wholly vague notion of physical possibility. Alternatively, if congruence is taken as primitive, it seems plausible that spatial discourse can be reconstructed so that there is no ontological commitment to spatial objects other than physical objects. Whether or not this relational thesis is ultimately defensible requires more elaboration of the relational theory, and investigation of its role in the formalizations of current physical theories. It is odd, considering the wide currency of relational theories, that this work has never been done. (shrink)

Doctoral students receive many kinds of assistance from faculty members, but much of this support falls short of mentoring. This paper takes the perspective that it is more important to find out what kinds of help students receive from faculty than to assume that students are taken care of by mentors, as distinct from advisors or role models. The findings here are based on both survey and interview data collected through the Acadia Institute’s project on Professional Values and Ethical Issues (...) in the Graduate Education of Scientists and Engineers. Continuing the dialogue: postcolonial feminist scholarship and Bourdieu — discourses of culture and points of connection Postcolonial feminist theories provide the analytic tools to address issues of structural inequities in groups that historically have been socially and economically disadvantaged. In this paper we question what value might be added to postcolonial feminist theories on culture by drawing on Bourdieu. Are there points of connection? In this paper we continue an ongoing dialogue that has as its goal the critical appraisal of theoretical perspectives on culture and health, in an effort to move forward scholarship on culture and health. We draw upon a programme of scholarship to explicate theoretical tensions and challenges that are manifest in the discourses on culture and health and to explore the possibilities Bourdieu's theoretical perspective offers for reconciling them. That is, we hope to demonstrate the need to move beyond descriptions (...) ‘of’ culture to an understanding of cultures as dynamic, and to show ways cultural practices create contexts that have the potential to foster or impede health.In our early research, largely undertaken in Canada's multicultural context, we sought to make visible the ways in which culture shaped conceptions of health and influenced health practices of immigrant groups. In recent years this focus has expanded to include populations that reflect the cultural and social diversity of our region. From the outset we attempted to move towards a conception of culture as negotiated, unifying, transformative and dynamic. While this position continues to hold appeal we are continually reminded that, despite our leanings towards constructivism, there is salience to the notion of culture as having enduring elements. It is this tension between the view of culture as embodied and enduring and culture as constructed and dynamic that we seek to examine.We explore whether Bourdieu's theoretical perspective offers promise for reconciling these apparently competing views. Using exemplars from our research we share insights that Bourdieu's work has offered to our analyses, thereby enabling us to move towards a view of culture that holds in tension these apparently contradictory positions of culture as both essence (albeit unstable, negotiated) and constructed. Tuberculosis (TB) is a potentially fatal infectious disease that, in Aotearoa New Zealand (NZ), inequitably affects Asian, Pacific, Middle Eastern, Latin American, and African (MELAA), and Māori people. Medical research involving genome sequencing of TB samples enables more nuanced understanding of disease strains and their transmission. This could inform highly specific health interventions. However, the collection and management of TB isolate samples for research are currently informed by monocultural biomedical models often lacking key ethical considerations. Drawing on a qualitative kaupapa (...) Māori–consistent study, this paper reports on preliminary discussions with groups of Māori, Pacific, and Afghan people in NZ, whose communities have been harmed by TB and TB stigma. Participants’ discussions highlight key concerns and meanings that ought to inform the development of guidelines and a more robust consultative process for the governance of how TB isolate samples are collected and used both retrospectively and in future research. We argue for ethical processes to be culturally nuanced and community-generated, flexible and meaningful, and situated in relation to the physical and symbolic effects of TB. We discuss the significance of Indigenous data sovereignty, rights, and kāwanatanga (governorship) in shaping a multicultural data sovereignty model. (shrink)

This article investigates the potential of fMRI to test assumptions about different components in models of complex cognitive tasks. If the components of a model can be associated with specific brain regions, one can make predictions for the temporal course of the BOLD response in these regions. An event‐locked procedure is described for dealing with temporal variability and bringing model runs and individual data trials into alignment. Statistical methods for testing the model are described that deal with the scan‐to‐scan correlations (...) in the errors of measurement of the BOLD signal. This approach is illustrated using a “sacrificial” ACT‐R model that involves mapping 6 modules onto 6 brain regions in an experiment from concerned with equation solving. The model's visual encoding predicted the BOLD response in the fusiform gyrus, its controlled retrieval predicted the BOLD response in the lateral inferior prefrontal cortex, and its subgoal setting predicted the BOLD response in the anterior cingulate cortex. On the other hand, its motor programming failed to predict anticipatory activation in the motor cortex, its representational changes failed to predicted the pattern of activity in the posterior parietal cortex, and its procedural component failed to predict an initial spike in caudate. The results illustrate the power of such data to direct the development of a theory of complex problem solving, both at the level of a specific task model as well as at the level of the cognitive architecture. To localize wheat ESTs on chromosomes, 882 homoeologous group 6-specific ESTs were identified by physically mapping 7965 singletons from 37 cDNA libraries on 146 chromosome, arm, and sub-arm aneuploid and deletion stocks. The 882 ESTs were physically mapped to 25 regions flanked by 23 deletion breakpoints. Of the 5154 restriction fragments detected by 882 ESTs, 2043 were localized to group 6 chromosomes and 806 were mapped on other chromosome groups. The number of loci mapped was greatest on chromosome 6B and (...) least on 6D. The 264 ESTs that detected orthologous loci on all three homoeologs using one restriction enzyme were used to construct a consensus physical map. The physical distribution of ESTs was uneven on chromosomes with a tendency toward higher densities in the distal halves of chromosome arms. About 43% of the wheat group 6 ESTs identified rice homologs upon comparisons of genome sequences. Fifty-eight percent of these ESTs were present on rice chromosome 2 and the remaining were on other rice chromosomes. Even within the group 6 bins, rice chromosomal blocks identified by 1-6 wheat ESTs were homologous to up to 11 rice chromosomes. These rice-block contigs were used to resolve the order of wheat ESTs within each bin. (shrink)

Recent research conducted in Australia shows that many oncologists withhold information about expensive unfunded drugs in what the authors of the study suggest is unacceptable medical paternalism. Surprised by the Australian results, we ran a version of the study in New Zealand and received very different results. While the percentages of clinicians who would prescribe the drugs described in the scenarios were very similar (73–99% in New Zealand and 72–94% in Australia depending on the scenario) the percentage who would not (...) discuss expensive unfunded drugs was substantially lower in New Zealand (6.4–11.1%) than it was in Australia (28–41%). This seems surprising given the substantial similarities between the two countries, and the extensive interaction between their medical professions. We use the contrast between the two studies to examine the generalisability of the Australian results, to identify influences on clinicians' decisions about what treatment information to give patients, and so the tendency towards medical paternalism and, more pragmatically, about how such decisions might be influenced. This article focuses on mirror self-recognition, the ability to recognize one's own image in a mirror. It presents the result of the first experiment on mirror self-recognition which showed that chimpanzees are able to learn that the chimps they see in the mirror are not other chimps, but themselves, as evidenced by self-directed behaviour. It reviews evidence for neural network for self-recognition and self-other differentiation and cites evidence that frontal cortex and cortical midline structures are implicated in self-recognition tasks. In this article, we present results from a literature review of intrinsic, instrumental, and relational values of nature conducted for the Intergovernmental Science-Policy Platform on Biodiversity and Ecosystem Services, as part of the Methodological Assessment of the Diverse Values and Valuations of Nature. We identify the most frequently recurring meanings in the heterogeneous use of different value types and their association with worldviews and other key concepts. From frequent uses, we determine a core meaning for each value type, which is (...) sufficiently inclusive to serve as an umbrella over different understandings in the literature and specific enough to help highlight its difference from the other types of values. Finally, we discuss convergences, overlapping areas, and fuzzy boundaries between different value types to facilitate dialogue, reduce misunderstandings, and improve the methods for valuation of nature's contributions to people, including ecosystem services, to inform policy and direct future research. © 2023 The Author(s). AndersonCharles Taylor, not assuming that Western secularity is without its own ethic, has described the moral impulses shaping modern lives as an "ethic of authenticity."1 Among the various marks one might discern in today's wider ethic is a desire to take seriously the particularities unique to (...) each human life. This means that any method of validly describing or prescribing for the modern person must show itself able of genuinely engaging with the particularities of an unrepeatable person enmeshed in a distinct and unique situation. Under such a predisposition, in so far as a moral matrix proves itself incapable of performing on such an individual and particular level, to that extent it is seen as artificial and even inhumane. By its modern hearer, such a morality is necessarily limited. It is perceived as a purely deductive morality that can merely apply general and undifferentiated norms without grasping with the variances in person and situations that lead to the potentially infinite variation of right values and actions. This pervasive, wider Western secular ethic of authenticity is worth considering the true locus of the rejection of the classical Christian affirmation of "absolute moral norms" or the "intrinsically evil acts" those norms prohibit. For his or her part, the Catholic moral theologian is left either attempting to explain how this classic Christian ethic is one that in fact bequeaths life and joy despite its apparent [End Page 1297] rigorism and insensitivity, or abandoning it altogether. The modern Catholic in turn would only be forced to consider their own decision making as "out of touch with real people" or to abandon their own intellectual tradition as having been so.At first glance, luminaries like Thomas Aquinas and others of his scholastic guild seem ill-equipped to help us. There is a serious reason for this, namely Aquinas's own Aristotelian method. According to the Aristotelian method, true scientific knowledge can be had only about universals, not particulars. This simple truth, which Aquinas embraced, entailed a focus on natures, essences, and causes, each of which were universals, and therefore capable of being meaningfully treated as part of any science, including sacra doctrina. Particulars mattered, but particulars simply needed to be perceived, since they were not per se universals.2While this method generally meant that Thomas did not speak at length of particulars, this itself is only a general rule. For there are places in his oeuvre where, like many Christian authors before him, Aquinas does give attention to individual cases of divine vocation in apparent contradiction of a divine commandment given to all. He treats the individual case of Abraham being called to sacrifice Isaac. But such treatments often fall only to biblical examples. Unfortunately, however, this means such cases are all very particular. For precisely that reason, if one wishes to move to reflections on modern human life, one needs to be willing to claim the same sort of inspiration that Abraham could.3 A second kind of particularity Aquinas [End Page 1298] addresses constitutes an oft-ignored portion of the secunda pars of the Summa theologiae (ST). Here he treats of charismatic gifts, states of life, and hierarchical offices.4 Yet here again is not the moral particularity for which the modern reader wishes. These do not touch on every human life, and Thomas gives only a generic understanding of these gifts, states, and offices. A third kind of particularity arises when Thomas is attempting to help confessors guide individual penitents. In such rarified places Aquinas distinguishes between two fora where a particular person was to be counselled. The first was a public forum, like a local diocesan marriage tribunal (forum exterius Ecclesiae), and the other was the forum of conscience (forum conscientiae), most obviously present in the context of sacramental confession and equipped with its own rules and guidelines.5 While the situations that arise out of such differentiation of fora are fascinating, the cases usually deal with agents in circumstances of rare moral perplexity and framing any wider moral matrix out of such... The assumption in current U.S. mainstream medicine is that birthing requires hospitalization. In fact, while the American College of Obstetricians and Gynecologists supports the right of every birthing person to make a medically informed decision about their delivery, they do not recommend home birth owing to data indicating greater neonatal morbidity and mortality. In this article, we examine the evidence surrounding home birth in the United States and its current limitations, as well as the ethical considerations around birth setting.

Anderson and his colleagues, is both a basic principle of human conduct and the most dominant strategy in recent years that individuals, organizations and countries use to pursue their ends. Although many scholars agree that a crisis of "over-leveraging" caused the financial crisis of 2008-2010, it has not been appreciated that an "over-leveraging" crisis has existed in American politics and the American family system as well. This book addresses the need for a "Leverage Mean" (...) (falling between the extremes of too much leverage and too little leverage) in the economy, politics, family life, and international relations. It identifies three different kinds of leveraging-bargaining, resource, and investment and provides an explanatory and normative theory which draws on the fields of economics, political science, sociology, history, psychology, international relations, law, and philosophy. Moreover, it shows how the dissolution of the Cold War, the dismantling of the modern family, and the rise of the Internet along with the deregulation of the financial services industry led to the diffusion of power which has made leveraging of the first importance for everyone. This book should be of interest to social scientists, philosophers, political theorists, public policy makers and politicians. David Anderson and his colleagues are the first to characterize and assess one of the major instruments of power of the contemporary era. This volume represents the best work on leverage since Archimedes and will be widely read and discussed. Allograft shortage is a formidable obstacle in organ transplantation. Xenotransplantation, the interspecies transplantation of cells, tissues, and organs, or ex vivo interspecies exchange between cells, tissues, and organs is a frequently suggested alternative to this allograft shortage. As xenotransplantation steadily improves into a viable allotransplantation alternative, several bioethical considerations coalesce. Such considerations include the Helsinki declaration’s guarantee of patients’ rights to privacy; political red tape that may select for undermined socioeconomic groups as the first recipients of xenografts; industry incentives in (...) xenotransplantation investments; conflicts of interest when a clinician supervises a patient as a research subject; the psychosocial impact of transplantation on the xenograft recipient, and the rights of animals. In this article we present a new approach to practical artificial intelligence (AI) ethics in heavy industry, which was developed in the context of an EU Horizons 2020 multi partner project. We begin with a review of the concept of Industry 4.0, discussing the limitations of the concept, and of iterative categorization of heavy industry generally, for a practical human centered ethical approach. We then proceed to an overview of actual and potential AI ethics approaches to heavy industry, suggesting that (...) current approaches with their emphasis on broad high-level principles are not well suited to AI ethics for industry. From there we outline our own approach in two sections. The first suggests tailoring ethics to the time and space situation of the shop floor level worker from the ground up, including giving specific and evolving ethical recommendations. The second describes the ethicist’s role as an ethical supervisor immersed in the development process and interpreting between industrial and technological (tech) development partners. In presenting our approach we draw heavily on our own experiences in applying the method in the Use Cases of our project, as examples of what can be done. (shrink)

AI has become the poster child for a certain kind of thinking which holds that some technologies can become objective, independent and emergent entities which can evolve beyond the control of their creators. This thinking is not new however. It is a product of certain philosophical ideas such as materialism, a common-sense world of objective and independent objects, a correspondence theory of truth, and so forth, which are centered around the pre-eminence of science, epistemology, and logical reasoning, among others, as (...) the supremely valid modes of engaging experience. This paper aims to critically examine the synthesis of the development program for AI to be found in the writings of computer scientist John McCarthy (1927–2011). McCarthy has been called one of the founding fathers of AI. First, some of the main themes which recur in his writings from as early as the mid nineteen fifties in the Dartmouth proposal, up to his late writings, are considered. A discussion of the goals that such a program implies, follows, in terms of society’s relation with AI based technology and the nature and purpose of AI itself. These implications are arguably related to social control and are also moral. Ultimately, McCarthy’s idea that AI is and can be built upon a paradigm of reasoning and logic so as to simulate a perfectly rational abstraction of human thinking is shown to be motivated by his view that AI systems should be controlled, as servants. This understanding of human thinking is questionable, however, given current research on human irrationality, as well as on the argumentative role of reasoning. And yet, the process of development toward the unachievable goal of controllable, rational, logical AI systems, has handily come to serve as a blind for increasing techno-corporate control of society, with techno-corporate interests displacing blame for devaluations and harms caused by their push to develop the technology to AI systems which they can conveniently ‘lose control of.’. We present a one per cent measurement of the cosmic distance scale from the detections of the baryon acoustic oscillations in the clustering of galaxies from the Baryon Oscillation Spectroscopic Survey, which is part of the Sloan Digital Sky Survey III. Our results come from the Data Release 11 sample, containing nearly one million galaxies and covering approximately 8500 square degrees and the redshift range 0.2 < z < 0.7. We also compare these results with those from the publicly released (...) DR9 and DR10 samples. Assuming a concordance Λ cold dark matter cosmological model, the DR11 sample covers a volume of 13 Gpc3 and is the largest region of the Universe ever surveyed at this density. We measure the correlation function and power spectrum, including density-field reconstruction of the BAO feature. The acoustic features are detected at a significance of over 7σ in both the correlation function and power spectrum. Fitting for the position of the acoustic features measures the distance relative to the sound horizon at the drag epoch, rd, which has a value of rd,fid = 149.28 Mpc in our fiducial cosmology. We find DV = at z = 0.32 and DV = at z = 0.57. At 1.0 per cent, this latter measure is the most precise distance constraint ever obtained from a galaxy survey. Separating the clustering along and transverse to the line of sight yields measurements at z = 0.57 of DA = and H =. Our measurements of the distance scale are in good agreement with previous BAO measurements and with the predictions from cosmic microwave background data for a spatially flat CDM model with a cosmological constant. © 2014 The Author Published by Oxford University Press on behalf of the Royal Astronomical Society. Gender, ‘race’, poverty, health and discourses of health reform in the context of globalization: a postcolonial feminist perspective in policy researchIn this paper, I draw on extant literature and my empirical work to discuss the impact of globalization and healthcare reform on the lives of women — those from countries of the South as well as of the North. First, I review briefly the economic hardships identified in different sectors of the population that have been attributed to how globalization is (...) now working. Second, I examine what these global processes mean for health, with particular focus on poverty, gender, racialization and health. Third, I reflect on how nurse scientists might develop research agendas in the 21st century that would foster social transformation and social justice for all people. The position taken here is not an indictment of globalization. Rather, I argue that globalization is a fact in all of our lives. There are positive aspects of globalization. There are also negative aspects which we must collectively address, given that the issues identified can have deleterious consequences for the world’s poor, women in particular. I suggest that, to construct knowledge for practice and praxis, research agendas of the future should be inclusive of subaltern voices. I argue that drawing on a postcolonial feminist epistemology might help us to define such agendas, and express the multilayered sociopolitical contexts of health and illness in advocacy with policy‐makers. These themes are: the unique environments of medicine and research; weighing the potential benefits of conflicts of interest against the risks; conflict of interest management; and the roles and responsibilities of diverse stakeholder groups.The Unique Challenges of the Medical and Research EnvironmentsThe reflections of the narrative authors draw attention to many of the unique environmental aspects of medicine and medical research that make physicians and researchers especially vulnerable to conflicts of interest. Conflicts of interest may be more ubiquitous in medicine than in any other industry (Kassirer, 2006). As the narratives demonstrate, conflicts of interest emerge across a range of health care delivery and research activities. Surveys have shown that 94 percent of physicians have some kind of relationship with industry (Campbell, Gruen, Mountford, et al., 2007). There is also evidence that physician interactions with medical industry begin early and are a regular part of a physician’s professional practice along the career continuum (Wazana, 2000).Medicine as a profession is distinct from banking or law. It is often said that medicine is an art; in treating patients, creative individualized solutions and gut instinct are as (if not more) important than the application of scientific knowledge. Uncertainty surrounds medical decision making. There is limited precision with respect to diagnoses and prognoses, limited data regarding the effectiveness of many treatments, and a range of patient values and preferences. Because of this uncertainty, rules about practice standards, especially with respect to avoidance and management of conflicts of interest, cannot be as explicit as they are in law or business. As demonstrated by the story presented by Sal Cruz–Flores, research and practice often intersect. And, there is no system in greater need of reform—and no system about which there are more divergent opinions as to what shape that reform should take—than the United States health care [End Page 91] system. The narratives of Tony Mikulec, Govind Nagaldinne, and David M. Zientek demonstrate how various aspects of the structure of healthcare service provision and third–party payer arrangements have the potential to harm patients. Conflicts of interest comprise more than relationships with pharmaceutical and medical device manufacturers.The narratives of Laura Jean Bierut and David M. Zientek also discuss examples of conflicts of interest that arise from factors beyond relationships with industry or the direct potential for financial gain. This may be somewhat unique to the medical milieu. Non-financial conflicts such as a desire for the prestige bestowed upon scientists who make key discoveries, a requirement to publish for tenure and promotion, the need to maintain one’s license or other privileges, or simply the pressure of a competitive environment can influence physician and researcher behavior. As noted by Bierut, more research on how to identify and manage non-financial conflicts is needed.The Potential Benefits and Harms of Conflicts of InterestConflicts of interest are usually presented in a negative light, particularly those that involve physician relationships with the pharmaceutical industry. To “have a conflict of interest” is to be in a situation or role where there is potential for personal interests to be prioritized over professional responsibilities. However, in terms of public perception, it seems that conflicts of interest have become almost synonymous with wrongdoing. The non-therapeutic physiological study, “Mechanisms of Deep Inspiration-Induced Airway Relaxation,” was designed to examine airway hyperresponsiveness in healthy individuals in order to better understand the pathophysiology of asthma. Participants inhaled hexamethonium, a (...) chemical ganglionic blocker that was not FDA-approved for use as specified in the experimental protocol. Risks of inhaling hexamethonium, including lung damage, had been reported in the 1950s and ‘60s. An investigation conducted after Roche's death determined that the study's principal investigator had failed to adequately search the medical literature; the protocol submitted for IRB review cited articles indexed in online literature databases that went back only as far as the 1970s.
